In the inspection and issuance of Phytosanitary certificates for air exports and on-site inspections, the following documents are required:
- Citizen Identification Card (Original 1 copy)

- Required for first-time service registration.
- Passport (Original 1 copy)
- Bring the original work permit in Thailand on the day of application submission.
- All copied documents must be certified as true copies.
- Corporate Certification Document (Original 1 copy)
- Must not be older than 6 months on the date of issuance.
- The applicant must bring the original corporate certification document on the day of application submission.
- All copied documents must be certified as true copies, along with the corporate objectives and the corporation's articles.
- Application for Phytosanitary Certificate (Form Por Kor 7) (Original 1 copy)
- List of Plants Attached to the Application for Phytosanitary Certificate (Form Por Kor 7.1) (Original 1 copy)
- Attachment to the Application for Phytosanitary Certificate (Form Por Kor 7.2) (Original 1 copy)
- Report of whitefly infestation inspection in the controlled plant production area for exporting controlled plants to the European Union (Original 1 copy)
- Health Certificate (Form Por Kor 11-1) (Original 1 copy)
- Attachment to the Application for Health Certificate (Form Por Kor 11.1) (Original 1 copy)
- Application for Amendment of Health Certificate Attachment (Form Por Kor 12) (Original 1 copy)
- Plant Pest Diagnosis Report from the Plant Quarantine Research Group (Original 1 copy)
- Import Permit (Copy 1 copy)
- Production Certification Document (Original 1 copy)
- Pack or deliver registered products.
- Bill of Lading or Air Waybill (Copy 1 copy)
- Letter of Credit or L/C (Copy 1 copy)
- Letter of Intent for Access Code (Original 1 copy)
- Power of Attorney (Original 1 copy)
- All copied documents must be certified as true copies.
- Copy of the Identification Card of the person granting power of attorney.
