For the issuance of Processing Statement (Annex IV) under the supervision of the Department of Fisheries, Ministry of Agriculture and Cooperatives, the applicant must prepare documents and follow the procedures below:

Supporting documents

• Aquatic animal health certificate; 
• Invoice identifying the number of aquatic animal health certificates; 
• Application form for health certificate with details of the product to be exported and date of issuing the health certificate.

Procedures for issuing Processing Statement (Annex IV)

• Inspecting completeness and correctness of documents;
• Considering the Raw Material Balance Stock (RMBS) and quantity for export, and inspecting the calculation of percentage yield, and details of the statement;
• Accepting and issuing the number of the application, and preparing the certificate in the Processing Statement Endorsement system;
• Issuing the statement number and printing the statement for signing;
• Signing the processing statement by the authorized person;
• Issuing the processing statement to the applicant;
